Palliative Care

 

Here at CDM we have two Palliative Care providers available to assist our patients. The main focus of our Palliative providers is care symptom management, care coordination, and communication between providers in order to align patient's/family’s treatment preferences with the best care plan from all available options of care. They mainly focus on patient populations with serious and/or progressive illnesses affecting their quality of life. 


Dr. Michael Hooda is a palliative care physician who has been working as an MD for 20 years.  He has a background in Hospice and also as a Hospitalist  most recently at Piedmont Henry Hospital. He strives to treat each patient individually based on the diagnosis and stage of disease. 


Dr. Urvi Patel is originally from the state of Gujarat, India. She completed her residency and fellowship from UAB campus in Montgomery and Main campus in Birmingham. Dr Patel’s primary specialty is Internal Medicine and subspecialty of Hospice and Palliative Medicine. Dr Patel has fellowship training and a Board certification in the field of Hospice and Palliative Medicine. 


What is Palliative Care?

Palliative care is specialized medical care that focuses on providing patients relief from pain and other symptoms of a serious illness, no matter the diagnosis or stage of disease. Palliative care teams aim to improve the quality of life for both patients and their families, while discussing the patient's goals of care.

Who benefits from Palliative Care?

Any patient who suffers from a serious illness no matter what stage the disease is in. This would be before Hospice care is needed or along with Hospice.

How often will the provider see the patient? 

Patients will typically be seen on quarterly rotations by the Palliative Care team, unless requested at other intervals. 

Will this replace their current PCP?

No, the provider will work alongside the current PCP to plan the best course of care for each patient. 

Psychiatric Care

 

We have several Psychiatric providers available to assist our patients. The main focus of our Psychiatric providers is to manage the patients mental health. By connecting with a mental health professional, residents can learn to more effectively cope with their emotions and work through trauma. Psychiatrists help to create a care plan that fits each individual patient's needs whether it be with therapy or medications. They work closely with the resident’s PCP and other specialists to determine the best plan of care. 


Dr. Alma Grewal is a Board-Certified Geriatric Psychiatrist. Originally from Florida, she obtained her bachelor’s degree from the University of Florida before going on to the Medical College of Wisconsin, where she completed medical school, residency, and a fellowship in geriatric psychiatry.  She spent the beginning of her career working in a variety of clinics at the Atlanta VA Medical Center.She has a particular interest in dementia care, not only in treating the behavioral symptoms but also providing a wholistic approach to include family and caregiver support. Dr Grewal services the Atlanta area. 

Bolanle Babajide-Gbenga, NP,  is a board certified Psychiatric Mental Health Nurse Practitioner. She received her Master of Nursing Degree and Bachelor Degree from The Walden University and Valdosta State University in Valdosta Georgia respectively. Bolanle serves both Skilled Nursing and Assisted Living Facilities within the Covington/Conyers areas. 

Chidi Osondu, NP is a psychiatric nurse practitioner. He completed his Bachelor of Science in Nursing at Clayton State University and earned his master’s degree at Georgia State University where he majored in psychiatry. He is very passionate about mental health and embraces a holistic approach of care involving the mind, body, and spirit. He is board certified by the American Nurses Credentialing Center (ANCC). Chidi serves the Atlanta area.


What is Psychiatric Care?

Psychiatry is the branch of medicine focused on the diagnosis, treatment and prevention of mental, emotional and behavioral disorders. A psychiatrist is a medical doctor (an M.D. or D.O.) who specializes in mental health, including substance use disorders. Psychiatrists are qualified to assess both the mental and physical aspects of psychological problems.

Who benefits from Psychiatric Care?

People seek psychiatric help for many reasons. The problems can be sudden, such as a panic attack, frightening hallucinations, thoughts of suicide, or hearing "voices." Or they may be more long-term, such as feelings of sadness, hopelessness, or anxiousness that never seem to lift or problems functioning, causing everyday life to feel distorted or out of control.

How often will the provider see the patient? 

Patients may be seen in a variety of different schedules based on what is best for their diagnosis. They may be seen weekly, bi-weekly, monthly, or quarterly.  

Will this replace their current PCP?

No, the provider will work alongside the current PCP to plan the best course of care for each patient. 

Physical Medicine & Rehabilitation

 

We currently offer PM&R services to patients in need. This medical speciality involved restoring a patients function who has been disabled by disease, disorder, or injury.


Dr. Yolanda Scott is a Physiatrist that is Board Certified by the American Board of Physical Medicine and Rehabilitation. With a background in Physical Medicine and Rehabilitation and Interventional Spine, she focuses on non-surgical musculoskeletal injuries, acute and chronic pain, degenerative changes as well as diagnostic testing for neuropathic conditions using electromyography testing. She also enjoys ultrasound-guided peripheral joint injections as well as trigger point injections for muscle tender points and acute muscle spasms. Dr. Scott’s training focuses on creating a collaborative treatment plan to improve a patient’s physical limitations. Her goals are to improve conditioning and provide an opportunity to return to an optimal lifestyle. During her free time she enjoys spending time with her family and friends, exploring the city of Atlanta, traveling, running in 5K events, and cooking.


What is PM&R Care?

Physical Medicine and Rehabilitation (or Physiatry), is a branch of medicine whose goal is to restore a patients functional ability and quality of life for patients that have become disabled or suffered injury. Physiatrists will direct and work alongside a rehabilitation team that may include PT, OT, SW, or ST.

Who benefits from PM&R Care?

Patients that suffer with chronic disease, pain, or disabilities looking to reduce pain, improve quality of life, recover/restore function, and learn how to use assistive devices.

How often will the provider see the patient? 

Patients may be seen in a variety of different schedules based on what is best for their diagnosis. They may be seen, weekly, bi-weekly, monthly, or quarterly.
